You can now enrol your child online using our online form.

Before you start the enrolment process, we recommend you get the following information handy - you can photograph and upload any physical files you have:

• Childs Details
• Photograph of child (digital)
• Medicare Details
• Private Health details (where applicable)
• Centrelink account information
• Medical documentation
